Terms of Service for Online Video Consultations

By requesting a teledentistry consultation you hereby agree to the following: that you wish to receive a teledentistry consultation with Dr. Tamburrino; in the absence of radiographs (x-rays), you understand that you may be asked to send photographs or other documentation if requested.  You understand that Dr. Tamburrino may be limited to what he is able to determine and diagnose remotely through video or telephone consultation, and might still be required to have an in-person visit to confirm any treatment plans. You also understand and agree that if you are experiencing pain or swelling that is life threatening, that you will call 911 or go to an emergency room. In addition, you understand this consultation may be recorded for clinical documentation purposes only and consent to this audio and/or video recording.
